1. Borrowing of books and publications

Secure and accomplish the Borrower's Logbook in the Counter. Indicate the title/subject of the books/ publications to be borrowed.
Service Provider
1. Evaluates the request. If available in the library, get the books/ publications from the shelf. If not available, refer the researcher to other concerned office/s where the data may be secured.
2. Gets the signed borrower's logbook and asks the ID card of client. Give the requested books/ publications to the researcher.

CIVIL REGISTRATION SERVICES
Evaluation on requirements of application of registration of authority to solemnize marriage of Solemnizing Officers (SO) before its submission to the Regional Office for processing and issuance of Certificate of Authority to Solemnize Marriage (CRASM)
Who May Avail of the Service:
Solemnizing Officers (SOs)
What are the Requirements:
1. Fill-out three (3) copies of OCRG-SO Form No.1 (application form), then subscribed and sworn the application form before a person authorized to administer oath, and affix one (1) documentary stamp on every copy of the application form;
2. Three copies of colored ID picture ( 2 x 2 ) with white background taken not more than a month ago from the date of application. Pictures should not be computer generated to preserve its quality. In cases the person is using glasses, it should be removed to have a clear image of the person. The back of the ID picture should contain the signature of the applicant;
3. A machine copy of appointment of Solemnizing Officer, either as priest, head, founder, bishop, pastor or minister of the religion or religious sect;
4. Proper endorsement/designation/recommendation from the head of religion or religious sect/Board of Trustees members to mention: the full name, nationality, complete residence address, name and location of the church, temple or mosque where the applicant regularly perform rites and indicate the extend of his territorial jurisdiction;
5. Certified True Copy of Certificate of Live Birth;
6. Alien Certificate of Registration ( ACR ) or Immigration Certificate of Registration (ICR) issued by the Commission on Immigration and Deportation ( CID ), in case the applicant is a citizen of a foreign country;
7. Certified True Copy of Certificate of Ordination issued by his/her respective church;
8. Certificate of Registration, Articles of Incorporations and by-laws, and updated General Information Sheet (G.I.S) certified by the Head of the religion or religious sect/ Securities and Exchange Commission.
9. Sworn statement duly notarized citing the full name, residence address, name and location of local church of founder (applicable for Corporation Sole type of SEC Registration);
10. Brief history of Church;
11.List of at least 200 church members stating their full name, residence address and signatures;
12.Certification from National Commission of Indigenous People (NCIP), if Indigenous People;
13.Certification from Commission of Muslim Filipinos, if Islam;
14. Photocopy of Endorsing Officer active CRASM/sworn statement duly notarized citing reasons of Endorsing Officer for non-renewal or not registering its authority to solemnize marriage;
15.Registration fee worth PhP 500.00
How to Avail of the Service:

The Solemnizing Officers (SO's) shall file their application for registration of authority to solemnize marriage at the Provincial Office (PO) of NSO where his/her place of assignment is located.
1. Receives and screens the application for registration of authority to solemnize marriage. Checks for the completeness of supporting documents.
2. Logs/enters information needed on the logbook for assignment of control number

1.Receives payment and issues Official Receipt

4.Processing of registration of authority to solemnize marriage at the RO.
5.If approved, the Regional Director (RD) signs and issue the Certificate of Registration of Authority to Solemnize Marriage (CRASM) and forwards the same to the PO for release; or
6.If pending application, the RO through the PO gives feedback/s to concerned SO for appropriate action.
